Kim and Harold Louie Family Foundation

This year, the Foundation is trying to reduce the amount of paper used.  Therefore, we would prefer that applicants submit the Adobe Acrobat version of the application by e-mailing the completed application to us at louiefoundation@gmail.com.  We would prefer that applicants download the Adobe Acrobat version of the recommendation form and e-mail the form directly to the two people providing the recommendations.  Those recommenders can then complete the forms and e-mail the recommendation forms directly to the Foundation.   All supplemental application materials, including transcripts, standardized test scores, tax forms, and proof of citizenship/legal residency, may be scanned and e-mailed to the Foundation.  Alternatively, you can mail paper copies of those documents to the Foundation.  If you are unable to use the Adobe Acrobat version of the application, you may submit an application on paper.  For your convenience, we also make available for download a version of the application form in Microsoft Word.
